		Our fees and payment schedule are both designed to provide quality care at an affordable price. We have contracts with many insurance companies that are prevalent among patients throughout Northwest Arkansas including plans provided by large local employers. We are also Medicare providers and offer a self-pay option for patients who do not carry insurance. We offer payment plans as well as discounts for payments that are received in full at the time of visit. For your convenience, we accept MasterCard, Visa, cash, and personal checks.

		For our patients with Medicare, Medicaid, Tricare and ARKids Our surgeons participate in the Medicare program. This means we will accept Medicare’s allowable payment and you will be responsible for the 20 percent co-insurance amount and any deductible that applies. You will be asked to pay your 20 percent co-insurance on the day of your visit. We do not take Tricare Prime. For our patients filing insurance In this age of managed care, it is hard to keep up with the different rules of individual insurance plans. We want you to have the most accurate and updated information possible when receiving our services so we ask that patients help us by taking responsibility for having specific information about their individual plan.
		If you have insurance coverage that requires an annual deductible and that deductible has not been met, you will be required to pay the allowable amount in full at the time of your visit. If you have met your annual deductible for the year, you will need to pay your co-insurance percentage which will vary according the standards listed in your insurance plan. This amount will be determined at the time of your visit. During your visit our insurance coordinator will call your insurance to verify your eligibility and benefits. If you are scheduled for surgery, the insurance coordinator will meet with you to go over the estimated portion you will be responsible to pay. You are asked to pay this amount in full or set up payment arrangements before you leave the office. As a courtesy, we will file your insurance requesting that all payment come directly to our office. If we have difficulty getting your insurance company to pay, we request that the patient assist our office in obtaining resolution of the bill or the balance may be turned over to the patient’s responsibility to resolve the balance on the bill . If you are unable to find the carrier on our list that appears on your card, please call the customer service number on the back of your insurance card to verify that the physician you will see is in-network. For our patients with Health Spending Accounts (HSA) or Health Reimbursement Accounts (HRA) A growing number of employers are using HSAs or HRAs for their employee benefit plans. We expect patients who have these plans to notify our office and to be prepared to pay whatever fees are appropriate in accordance with their plan. For our patients with Medicare, Medicaid, Tricare and ARKids Our surgeons participate in the Medicare program. You will be asked to pay your 20 percent co-insurance on the day of your visit. For our patients using the self-pay option (no insurance) Self-pay patients are asked to pay in full at the time services are rendered. Any exception to this policy must be approved at the time your appointment is made. A discount will be given to self-pay patients who pay their bill in full at the time of service. Do you have a Flex-Spend Account (FSA)? We offer receipts for all payments that can be turned in to your flex-spend account provider for reimbursement. Please ask before the receipt is made if your provider requires any specific information.
